To require last name in the loyalty page, in the Orda Admin, go to Marketplace > Loyalty Screen > Disable the field "Require only first name". Publish a new version. Logged users don't need to login again so it will only effect new users.

Fixed, starting with beta testing. This feature will require already logged in customers to log out.
